Some specialist and made-to-order products require additional information before an order can be accepted by the supplier. This may include measurements, sizing details or information about how the product will be used.

Please download and complete the relevant form or questionnaire below, following any instructions provided. Once completed, return the form to Fledglings as requested so that we can check the details and progress your order.

Providing accurate information is particularly important for products where fit and sizing are critical, such as specialist car seats, head protection and other made-to-measure or custom products.
